The type of personal information we collect 

We currently collect and process the following information:

• Personal identifiers such as name, address, email address and telephone number.

How we get the personal information and why we have it

To access our service, you will have contacted us directly and we will collect information from you. This will include details such as your name, address, contact details and GP details. We use the information that you have given us in order to provide you the service that you requested, to allow us to liaise with you via email or text message and to ensure we are offering you the appropriate support or signposting you to alternative services if deemed more suitable. We may use your information to contact you about other services we can offer.  If engaging in coaching, we will also collect information relating to your emotional wellbeing.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

1. Your consent. You are able to remove your consent at any time. You can do this by contacting support@adaptadhd.co.uk

2.  We have a contractual obligation.

3. We have a vital interest.

4. We have a legitimate interest.

What information will we share?

At Adapt ADHD, we know the importance of your personal information. We, in accordance with our professional bodies, will ensure that supervision is undertaken as directed. When seeking coaching supervision, we will not disclose any personal data such as your name and the supervisor will also be bound by confidentiality. 

We will not share your information with any other organisation for marketing or commercial purposes or to third parties.

All information given to us is treated as confidential between you and Adapt ADHD. The only time that we will share information with services outside the arrangement above is due to legal reasons or safeguarding reasons. We may be required to disclose personal data to statutory bodies (NHS services or Social care etc) if we have significant concerns of harm to yourself or others. If we do have concerns, we will endeavour to discuss this with you before sharing.

How we store your personal information 

Your information is securely stored. We keep your personal details such as name, address and contact details on a clinical system called Semble. We only store the information for as long as is necessary to provide you a service and for service improvement. We will only keep your personal information for as long as it is necessary for the purposes set out in this privacy notice, unless a longer retention period is required or permitted by law (such as tax, accounting, or other legal requirements).

When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymise such information, or, if this is not possible (for example, because your personal information has been stored in backup archives), then we will securely store your personal information and isolate it from any further processing until deletion is possible.

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

How to complain
